Out-of-Network Agreement: 

If your insurance plan is not listed, you may be eligible for out-of-network reimbursement for services by your insurance company. Please call for consultation to discuss in further detail.

Private Pay Fees:

First Session – intake and evaluation: $175.00 (1 hour)

55 minute sessions - $150.00

Less than one hour per 15 minutes : $45.00 ( this may include a consultation, provider collaboration, or other task outside of normal session time)

Sliding Scale Arrangements:

Firm Foundation Therapy PLLC is committed to offering a handful of sliding scale slots for those who do not have insurance that is in-network. Please contact us for further inquiry and to see if a sliding scale spot is available. 

Cancellation Policy: 

A $150 cancellation fee will be charged for appointments canceled less than 24hrs prior to the start time of the appointment. The same fee will be charged for no show/no call appointments. Being more than 10 mins late to an appointment, without any communication with the therapist will automatically be counted as a no show appointment and the fee will be charged.
